After you have finished, if you change your mind or need to tell us about changes to your child’s medical history, do not complete another consent form. Please

to tell us about any changes.
Please note this consent will last until your child leaves school. It is your responsibility to notify the school aged immunisation team if there are any changes to your child's medical history or if you change your mind about the vaccination. The school age immunisation team will continue to contact you annually whilst your child remains unvaccinated.
